Job Description
I. Position Summary
The Case Manager is a competent professional member of the management team and responsible for providing for the case management at Pendleton Place. The Case Manager shall support the agency’s mission, adhere to its values, maintain the confidential nature of the agency’s services and perform the duties of the position in a professional manner. This position operates under the supervision of the Assistant Director.
The primary mission of Pendleton Place Children’s Shelter is to provide services to abused, abandoned, neglected and dependent children who meet the eligibility criteria. The primary purpose of the Case Manager position is to manage and/or coordinate the services provided to the children so that the employees and stakeholders can best meet the children’s needs.
II. Primary Duties and Responsibilities of this Position (these tasks are illustrative only; to carry out the day-to-day functions of the job, other duties may be performed. - Exercises good customer services techniques, such as listening, effective communications and problem solving.
- Communicates professionally with a variety of individuals and groups including public defender, solicitor’s office, attorneys, public, court officials, law enforcement, community leaders, clients, agency personnel, DSS caseworkers, victims, families, schools, volunteers, etc.
- Screens and interviews clients; assesses client needs and assigns or refers them to the appropriate resource; mediates problems for clients; prepares and maintains the appropriate paperwork and documentation for each case file.
- Reviews/analyzes and evaluates reports, information, and programs for individual clients and, in a team environments, develops an appropriate action plan for client developmental growth.
- Composes and prepares correspondence or provides information to the appropriate placing agency. Prepares a variety of reports, records and forms on a daily, weekly and monthly basis within the deadlines established by the Assistant Director.
- Maintains progress reports on cases; follows up with clients and referral sources.
- Supervises programs and employees as required.
- Enroll residents in school and be a liaison between Pendleton Place and the schools.
- Maintain paper and electronic client records in accordance with agency policy.
- Monitor adjustment and progress of each child.
- Prepare and discuss monthly summaries of each resident’s progress with DSS caseworker.
- Attend court proceedings, school appointments, IEP and Foster Care Review Board meetings.
- Coordinate visitation, home visits, appointments and services from other agencies.
- Assist in transportation for appointments, and supervised visits.
- Participate in plan of care and reviews for children in your unit.
- Provide support to childcare workers in dealing with problems (family, school, behavior).
- Develop working relationships with DSS workers, Guardian ad Litem’s, therapists, schools, as well as the families and children on your caseload.
- Complete reports as needed for program evaluation, client assessments, and documentation. (E.g. Application Forms, Entrustment Agreements, Plan of Cares, Progress Summary Notes, etc).
- Assure consistent contact and communication with each resident in care.
- Coordinate discharge planning with DSS caseworkers and complete discharge summaries.
- Performs other duties as required.
IV. Qualifications
- A Baccalaureate degree from an accredited university or college in Psychology, Social Work, Early Childhood Education, Child Development, or a related field to include, but not limited to Criminal Justice, Rehabilitative Counseling, Elementary or Secondary Education; or a holder of a Baccalaureate degree in another field with additional training (a minimum of 45 documented hours of training that could include undergraduate or graduate courses, workshops, seminars, conferences in issues related to child development, children’s mental health issues and treatment) in one or more of the above disciplines and who has a minimum of three years of experience working with the population which is to be served.
- Will have satisfactory criminal history and background check determination. (Child Abuse and Neglect Central Registry check, Sex Offenders Registry check, Motor Vehicle Registration check and a satisfactory determination or preliminary records check application.)
- Comprehensive knowledge of case management and counseling techniques.
- Thorough knowledge of community resources and social services agencies.
- Ability to investigate, collect, compiles and presents a variety of information and facts clearly and concisely.
- Knowledge of supervisory skills
- Ability to exercise judgment and discretion in interpreting and applying various procedures and guidelines.
- It is understood that most of the duties listed are time sensitive. Case managers must be able to perform all duties in the time frames required by DSS Contracts, DSS Licensing or internal guidelines established by the Assistant Director or Executive Director.
- Ability to deal with persons in a highly stressful situations.
- Will have or will obtain CPR/First Aide Certification.
- Physical ability to lift and carry 50 pounds.
- Ability to understand, to enjoy, and to be comfortable with children.
- Ability to serve as a role model for residents.
- Ability to work cooperatively as a team member.
- Possess valid South Carolina driver’s license.
